What to do:

• Before your appointment, you will receive a summary of your child’s profile.

• Please check the information carefully and either confirm it is correct or make any necessary changes using a red pen.

• The completed form must be returned before leaving the school premises to the school office.

Keeping your child’s information up to date ensures we have the correct emergency contacts, medical details, and other essential information needed to support their wellbeing and safety.
